All incoming Freshman are required to submit: 

Student Emergency Information - Due By March 31, 2023

Certificate Of Child Health Exam  - Due by May 1, 2023                              
Health examination with proof of immunizations prior to the first day of school. This health examination must be completed by a MD, DO, APN or PA on or after 8/16/22.  The Health History section on page 2 must be completed and signed by the parent/guardian.

Dental Examination - Due by May 1, 2023
Dental examination must be completed by a licensed dentist on or after 11/15/22.
